Investigate and Evaluate Health and Safety Incidents and Complaints in the Workplace
ProQual Level 3 NVQ Certificate in Occupational Health and Safety
Workplace incidents, near misses, dangerous occurrences and health and safety complaints provide valuable opportunities to identify weaknesses in health and safety arrangements and prevent recurrence. This mandatory unit of the ProQual Level 3 NVQ Certificate in Occupational Health and Safety focuses on developing the practical skills required to investigate events professionally, analyse findings and recommend improvements.
Learners completing this unit will demonstrate competence in gathering information, conducting investigations, identifying root causes, preparing reports, developing recommendations and ensuring that improvements are implemented and reviewed.
Unlike traditional classroom-based qualifications, the ProQual Level 3 NVQ Certificate in Occupational Health and Safety is a competency-based qualification assessed through workplace evidence. This means learners demonstrate their ability to apply health and safety knowledge in real workplace situations rather than sitting written examinations.
This unit is particularly relevant for:
Supervisors and Team Leaders
Health and Safety Coordinators
Health and Safety Representatives
Operations Supervisors
Team Managers with health and safety responsibilities
Individuals progressing towards Technician Membership of IOSH (TechIOSH)
By successfully completing this unit, learners will demonstrate their ability to investigate workplace incidents and complaints effectively and use findings to improve health and safety performance.
What Does This Unit Mean?
This unit is about investigating workplace incidents, accidents, near misses, dangerous occurrences and health and safety complaints.
The aim is to identify what happened, why it happened and what can be done to prevent similar incidents from occurring in the future.
The unit also requires learners to analyse investigation findings, develop practical recommendations and ensure that improvements are implemented and reviewed.
In simple terms, this unit is asking:
“Can you investigate workplace incidents and complaints, identify the causes, recommend improvements and help prevent recurrence?”
What Competence Does This Unit Require?
You are expected to demonstrate that you can:
Gather information from appropriate sources during an investigation.
Conduct workplace investigations in a fair, systematic and professional manner.
Interview and communicate effectively with people involved in incidents and complaints.
Identify immediate, underlying and recurring causes.
Prepare investigation reports.
Develop practical recommendations to improve health and safety.
Ensure recommendations comply with legal and workplace requirements.
Present recommendations to the appropriate people.
Develop action plans and review implementation.
The focus is not on simply completing an incident form.
The focus is on demonstrating that you can investigate incidents effectively and use the findings to improve health and safety performance.

Important Note for Learners
The competence requirements outlined above describe the practical workplace activities, systems, processes and records that may be used to demonstrate competence against this unit.
As this is a competency-based qualification, learners are expected to demonstrate that they can apply their health and safety knowledge and skills within their own workplace or working environment.
In addition to the competence-based assessment criteria, this unit also contains a number of knowledge-based assessment criteria. These are typically assessed through:
Written responses
Professional discussions with the assessor
Assessor questioning
In most cases, the knowledge-based assessment criteria relate directly to the same workplace activities, systems and processes described above. Learners will therefore often find that the workplace evidence gathered to demonstrate competence also helps them prepare for and satisfy the knowledge assessment requirements.
